Rehabilitation Counsellor

Men’s Mission has an opening for a Full-Time Rehabilitation Counsellor. This role will provide support in participants' housing search, crisis counseling as needed, and ongoing support. The position reports to the Direct Care Supervisor.

Duties and Responsibilities:

  • To ensure clients are “paper ready” (holding a current VI-SPDAT, personal identification, and confirming income security) within the Housing First model.
    To match participants to housing agencies and assist client in housing search.
  • Provide support to residents/clients in the development and achievement of personal goals and objectives.
  • Develop one-time and ongoing educational /informational /recreational sessions for residents/clients in consultation with other staff.
  • To provide applicable referrals to community agencies/resources with follow-up and advocacy.
  • Facilitate efficient and positive service connections within the Men’s Mission and Mission Services of London as well as between Men’s Mission and other community agencies.
  • Maintain ongoing communication with staff regarding relevant resident/client information for developing Plans of Care and ensuring a safe and positive stay in shelter.
  • Tracking client’s financial circumstances, determining eligibility and monitoring changes and pay-for-stay requirements.
  • Maintain resident/client files documenting process of engagement, establishment of goals and objectives and progress being made.
  • Provide timely, concise and accurate documentation of resident/client centered activities in accordance with required information.
  • Provide monthly breakdown of caseload numbers and other reports as requested.
  • Develop positive service linkages with community agencies/resources, resulting in seamless service to residents/clients.
     

Qualifications:

  • Diploma or degree in Human and/or Social Services.
  • 2-3 years’ experience in Case Management/Rehab Counselling, (residential setting preferred)
  • Able to work effectively with diverse male populations.
  • Skilled in crisis intervention, conflict resolution, and use of diagnostic tools.
  • Strong critical thinking and analytical skills

Position Details:

  • Full Time – 40 hours per week
